CARMAN FAIR - DEMOLITION DERBY

PLEASE READ CAREFULLY. WARNING: BY SIGNING THIS, YOU GIVE UP THE RIGHT TO SUE. WAIVER OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT:

I DO UNDERSTAND and acknowledge that there are particular hazards and risks which are directly or indirectly related to this activity / event and / or which are either associated with this (activity/event) and/or are inherent in their nature.

I DO UNDERSTAND and acknowledge that I may sustain personal, bodily injury and may sustain property loss or damage through my participation in this (activity/event).

I DO UNDERSTAND the risks and dangers involved in this (activity/event) and willingly and voluntarily assume them at my own risk.

I DO UNDERSTAND that by signing this agreement, I give up my right to sue and I release the DUFFERIN AGRICULTURAL SOCIETY and all sponsors of the (activity/event) from any claims, costs, causes of action or personal injuries that may arise out of my involvement in this (activity / event).

THE TERMS OF this agreement shall be binding upon me, my heirs and personal representatives.

DEMOLITION DERBY ENTRY FORM

● ENTRY FEE: $60. (for 1 driver+ 1 pit crew)
● Additional pit crew entry fee $15 each (Maximum of 1 Driver 2 Pit crew)
● Pit crew names must be included with entry.All Entries must be prepaid by e-transfer to carmfair@mymts.net with the memo "Demo Derby Entry Fee".
● Entries received after the deadline will be charged $20 additional fee.
● ENTRY DEADLINE - 4 P.M. Wednesday, July 6th, 2022.
● Drivers and crew must be in the pits by 11am and start at 1pm sharp on July 9, 2022. Entrance is at the north side of the Carman fairgrounds.
I have read the rules, accept the decision of the judges as final, and elect to use the grounds and track in its present condition.
